Antifungal Exposure and Resistance Development: Defining Minimal Selective Antifungal Concentrations and Testing Methodologies

چکیده

This scoping review aims to summarise the current understanding of selection for antifungal resistance (AFR) and compare contrast this with antibacterial resistance, which has received more research attention. AFR is an emerging global threat human health, associated high mortality rates, absence effective surveillance systems few alternative treatment options available. Clinical well documented, additional settings increasingly being recognised play a role in evolution spread AFR. The environment, example, harbours diverse fungal communities that are regularly exposed micropollutants, potentially increasing risk. direct application effect concentrations azole fungicides agricultural crops incomplete removal pharmaceutical antifungals wastewater particular concern. Currently, environmental risk assessment (ERA) guidelines do not require agents terms their ability drive development, there no established experimental tools determine selective concentrations. Without data interpret antifungals, our effectively inform safe thresholds severely limited. In review, potential methods generate concentration proposed, informed by approaches used minimal Such can be considered development regulatory aim reduce

Antifungal susceptibility testing.

Antifungal susceptibility testing has been in routine use now for more than 15 years and has become a useful tool for clinicians who are faced with difficult treatment decision. Although most clinicians order susceptibility testing, much confusion still exists regarding the use of the results. Sufficient data have been generated to determine susceptibility trends for specific fungi against spec...

Personalized antifungal susceptibility testing.

Introduction In recent years, the remarkable increase in the number of infections caused by fungal pathogens, especially in immunocompromised people, has stimulated interest in medical mycology in general and in antifungal susceptibility testing in particular. 1,2 In-vitro procedures for determining the activity of drugs against mould and yeast isolates involve clinicians in the choice and moni...

Mechanisms of antifungal resistance

The many drugs that are available at present to treat fungal infections can be divided into four broad groups on the basis of their mechanism of action. These antifungal agents either inhibit macromolecule synthesis (flucytosine), impair membrane barrier function (polyenes), inhibit ergosterol synthesis (allylamines, thiocarbamates, azole derivatives, morpholines), or interact with microtubules...

Antifungal drug resistance.

The increasing incidence of invasive fungal infections is the result of many factors, including an increasing number of patients with severe immunosuppression. Although new drugs have been introduced to combat this problem, the development of resistance to antifungal drugs has become increasingly apparent, especially in patients who require long-term treatment or who are receiving antifungal pr...

Dermatophytes: host-pathogen interaction and antifungal resistance.

Cutaneous mycoses are among the most common infections in humans and have become an important public health issue because they cause invasive infections in immunocompromised patients. During the infectious process, dermatophyte-host interactions trigger specific metabolic adaptations that allow the pathogen to adhere to and penetrate the host tissue, scavenge nutrients, and overcome the host de...
